Key Components & Preparation


Process

Applicants must complete form FAA 8500-8 via MedXPress before the appointment. The confirmation number generated after completing the above application is necessary for the Aviation Medical Examiner (AME) to perform the exam and issue a certificate, if all criteria are satisfied.


Examination

Includes checking blood pressure, urine (for glucose/protein), vision (20/40 or better required), and a general physical exam.

 

Preparation

Be rested, avoid high-sugar meals or caffeine, and bring a list of medications and any relevant paperwork related to recent medical history.

 

Cost

  • First Time Applicants (All 3 classes) - $200.00
  • Renewals (All 3 classes) - $150.00
  • Waggoner's Computerized Color Vision Test included for first time applicants.

 

Additional Documentation

If you have a FAA Authorization letter which was issued for any condition, please bring a copy of the authorization letter to the exam.

If you have any medical conditions, or medications which have been listed in your application, you may need additional paperwork from your treating physician, including blood test results as required by FAA guidelines.

 

Please call ahead if you are not sure about the paperwork required for your medical conditions or prescription medications which you are currently using. Some of the common medical conditions and their requirements are listed below.

 

Hypertension

Provide a current, detailed clinical progress note generated from a clinic visit with the treating physician or specialist no more than 90 days prior to the date of your AME exam.

 

Obstructive Sleep Apnea

A certificate may be issued for an applicant currently on an AASI for OSA, if the necessary documents are provided.

Applicants should provide:

  • An authorization letter granted by the FAA
  • A signed airman compliance with treatment sheet attesting to absence of obstructive sleep apnea symptoms and continued daily use of prescribed therapy
  • A current status report from the treating physician indicating that obstructive sleep apnea treatment is still effective

Applicants should also provide a copy of the cumulative annual PAP device report which shows actual time used. Target goal should show use for at least 75% of sleep periods and an average minimum of 6 hours use per sleep period.


If the applicant is being treated for obstructive sleep apnea but has not previously reported it to the FAA, the specification sheet A will be provided, which will list the required documents that need to be provided to the FAA within 90 days.

 

Low Testosterone on Replacement Therapy

If you are treated with topical testosterone only, no additional paperwork would be required.

 

If you are treated with oral or injectable testosterone, pellet therapy, anastrozole, HCG injections, or clomiphene citrate, a current, detailed clinical progress note generated from a clinic visit with the urologist, endocrinologist, or treating physician no more than 90 days prior to the AMA exam is required.

 

The progress note should indicate that the condition is stable without side effects. The applicant should have no history of cardiovascular disease, thromboembolic events, or prostate cancer. Applicants should not require treatment with phlebotomy.

 

A complete blood count should be performed within the last 90 days prior to the exam showing a hematocrit value of 54% or less.

 

Hypothyroidism

Provide a current, detailed clinical progress note generated from a clinic visit with the treating physician or specialist no more than 90 days prior to the date of your AME exam. 

 

Applicants should not have any active symptoms and signs of thyroid dysfunction and should be treated with acceptable medications such as Synthroid, Levothyroid etc.

 

A blood test for TSH with a result of less than 10 (uIU/ml or mIU/L) performed within the last 1 year should be included.

 

Important Note on Clinical Progress Notes

Please note that a current, detailed clinical progress note must include:

  • A summary of the history of the condition
  • Current medications and dosages
  • Side effects (if any)
  • Clinical exam findings
  • Results of any testing performed
  • Diagnosis
  • Assessment and plan
  • Prognosis and follow-up as appropriate

 

Final Reminder

Please make sure to bring all relevant documents to the AME exam, to avoid unnecessary delay in receiving the medical certificate.
