You may email us a request for an appointment and time, and we will send you an email confirmation or an alternate time available. All email requests must be at least 48 hours in advance. If you need a sooner appointment, please call.

We cannot answer medical questions or do refills over the internet at this point. We can’t diagnose or treat you or prescribe medications without an exam and office visit. We will do everything possible to accommodate your time and to make your appointment quick and easy.
